  1. I absolutely understand and accept that such procedure(s) is a process, often requiring a follow-up application (depending on the service). For all PMU services: a follow-up application of color may be needed to achieve desirable results and 100% success cannot be guaranteed.

  2. I acknowledge that obtaining Permanent Makeup is my choice alone. The application of Permanent Makeup will result in a permanent change to my appearance and that needles and pigments will go into my skin using only sterile, disposable, single use needles and supplies. No representatives have been made to me as to the ability to later restore the skin involved in Permanent Makeup to the original condition and can be costly to remove.

  3. I understand that the Permanent Makeup site usually takes 2 weeks or longer to heal. I understand that an infection is always possible as a result of a Permanent Makeup application and I agree to take full responsibility to care of the procedure site as directed by my technician. Possibilities may include: redness, minor bleeding, swelling, tenderness, allergic reaction, hypertrophic, keloid formation, inconsistent color and/or spreading of fanning of pigment. Tattoos should be considered permanent; that it can only be removed with a surgical procedure; and that any effective removal may leave permanent scarring and disfigurement. I understand The Tattoo inks, dyes, pigments have not been approved by the Federal Food and Drug Administration (FDA), and the health consequences of using the products are unknown.

  4. I understand that the actual color of the pigment may be modified after the procedure due to the tone and color of my skin

  5. I understand that I am responsible for picking and approving the shape drawn on by my technician. I understand the shape I will approve drawn on by my technician is what I want, and the procedure will not start until I approve the outlines desired shape.

  6. I understand that the positioning of my procedure can be affected if I elect to have cosmetic surgery, Botox, or fillers.

  7. I understand that this procedure is permanent in nature but will fade over time, this fading can alter the original color, and that this determines that it is time for a touch-up which should be done every year or possibly sooner to maintain the integrity of the color. Permanent Makeup is a commitment that must be kept up with.

  8. I understand that peeling and flaking will occur and even loss of color during the healing process. Procedure should be done more that 2 weeks prior to important events.

  9. I agree to release and forever discharge and hold harmless the technician (Courtney), all employees, contractors, and the management of the Permanent Makeup business (Sunkissed by Courtney) for any and all claims of negligence, damages, or legal actions arising from or connected in any way with my Permanent Makeup procedure.

  10. I understand a touch up is recommended any time after 8 weeks from initial first-time procedure to ensure complete satisfaction and final results. I understand touch up fees are based on timeframe (Please refer to website for current pricing).

  11. I understand that before and after pictures may be taken for our records and may be used for educational or advertisement purposes. Pictures may also be used for social media purposes. I understand I cannot decline having pictures taken or used by Sunkissed by Courtney.

  12. I understand that it is my responsibility to book my touch ups accordingly and understand that touch ups are based on timeframe. (Prices can be found on our website)

Sunkissed by Courtney Policy

  1. Please make sure you are completely satisfied with the outline before the procedure begins and completely satisfied with the outcome of the procedure before leaving your appointment. If corrections need to be made once you have left the appointment, you must wait 8 weeks until the skin is fully healed and a touch up fee will apply. Touch ups are based on timeframe from last visit.

  2. There is a 10-minute grace period and if you are running later than 10 minutes for your appointment time you will be rescheduled and you will have to put down a new deposit to reschedule.

  3. You may reschedule or cancel your appointment 2 days in advance to be able to reschedule without placing a new deposit.

  4. No call/ No shows will have to place a new deposit to reschedule and may be subject to a no appointment block list.

  5. We have the right to refuse service.

  6. A second session is recommended any time after 8 weeks from new initial procedure to ensure desired final results. Second session pricing will be determined based on the timeframe you return (this is not included in your initial procedure price; it is an additional fee). Please review website for pricing. Please book any and all appointments online. On a rare occasion, a client may need a third or fourth session due to having a skin type that is more resistant.

  7. Any touch up sessions that you are returning to after initial procedure are not to change the shape or color received in initial appointment. Touch ups are to perfect what was already done to complete the service or maintain it.

  8. Photos taken before, during, and after your procedure may be posted on our

    website and your Artist's social media outlets for marketing purposes. We have rights to any pictures taken.

  9. We cannot guarantee 100% successful results to our clients because everyone's skin takes on Permanent Makeup differently. There are also no refunds as we have carried out any and all services.
